I Declare that all Livestock are in good health, with ZERO Bio-Security concerns

I confirm I am the legal owner of the livestock consigned to Skyview Processing.

I acknowledge that as the owner of consigned livestock, I am required to ensure my animals are tagged/identified as required by law.

I consent to my information being shared with Bio-security and other agencies as required in the event of an Animal Disease outbreak or other such bio-security concern, namely transport type, dates and property of origin.

I acknowledge it is my responsibility to move animals from my PIC to the Consigned PIC of MCPC0473 for processing.

I understand that failure to provide necessary paperwork on day of delivery will result in a fee of $200 being applied to my account and my livestock will not be processed and will be returned at my own expense.
